Obtaining preliminary clearance
The first step to obtaining the permit is to fill out a paper called “Application Form for Registration of a Medical Establishment.” This form is available in Arabic and English. The form includes the following sections:
Medical establishment application form: This form asks about the type of business, location, ownership, the nature of the business, the address of the medical establishment, the type of business, whether the establishment is to be owned by one person or jointly by two or more people, the legal status of the establishment and the name of the owner.
License application form: You are required to fill out this form in order to obtain a license for operating the establishment. The license is issued for a period of three years.
Approval from the Department of Health and Human Services
To get your license you must first fill out an application form and submit it to the DHA. Before you do so, make sure that you have received approval from DED. You can get this clearance from DED. After you have filled out the application form, you must include a feasibility study for your clinic / business plan. This is an important document because it provides the DHA with the details that they require. You must include these information in the feasibility study and provide proof of them. You will also have to pay a fee to apply for a license. The DHA will ask you to submit these documents as well. These documents include the feasibility study, business plan and license application.
